Pendant le contrat actuel, notre recherche, qui est de nature translationnelle, a été centrée sur la physiopathologie et le soin des formes génétiques et acquises des lipodystrophies (LD). Notre équipe a acquis une reconnaissance au niveau national et international du fait des études que nous avons conduites dans ces domaines. Nous analysons les différentes composantes de ces maladies en utilisant des approches translationnelles : recherche des mécanismes physiopathologiques en jeu, conséquences du vieillissement des patients, recherche des facteurs génétiques, de nouveaux outils diagnostiques et pronostiques, évaluation de nouvelles approches thérapeutiques et amélioration des pratiques cliniques. Une des forces de notre équipe est notre capacité à développer de nouvelles recherches aussi bien fondamentales que cliniques grâce à l’expertise complementaire des scientifiques et des cliniciens qui y travaillent. De plus, notre équipe a développé de nombreuses collaborations fructueuses avec les autres équipes du CRSA, avec l’IHU ICAN et avec des équipes ayant des expertises complémentaires au niveau national et international, nous permettant ainsi de renforcer et de valoriser nos programmes de recherche.
Nous travaillons sur quatre grandes thématiques dans le contexte des lipodystrophies génétiques et acquises, de leurs adaptations métaboliques au niveau systémique, des conséquences du vieillissement et de leur impact sur la fertilité et les fonctions reproductives.
L’étude de ces syndromes rares de LD génétique bénéficie de l’opportunité unique de rassembler au sein de l’équipe des expertises en génétique, en biologie cellulaire, en clinique en réalisant des investigations physiopathologiques sophistiquées, et de l’expertise concernant la prise en charge des patients avec, en particulier, le développement d’outils d’éducation thérapeutique. Le groupe occupe au niveau national la position d’expert dans ces pathologies, ce qui a été concrétisé par la création en 2017 d’un centre de référence des maladies rares (CRMR) PRISIS (pathologies rares de l’insulino-sensibilité et de l’insulino-sécrétion) coordonné par C. Vigouroux. Ce centre a été renouvelé en 2023 sous sa direction. Ce groupe fait aussi partie du consortium Européen des Lipodystrophies (ECLip) et du réseau Européen des maladies rares « Endo-ERN », dans le groupe thématique des « Genetic disorders of Glucose & Insulin Homeostasis ».

Ce groupe a travaillé depuis sa création sur la thématique des syndromes lipodystrophiques associés au VIH et à son traitement. Le phénotype des personnes porteuses du VIH recevant un traitement antirétroviral a évolué de façon importante en fonction de la nature des molécules thérapeutiques reçues par les patients. Actuellement, il se caractérise par une lipohypertrophie soit généralisée soit localisée au niveau tronculaire. A ces atteintes, s’ajoute un vieillissement accentué du tissu adipeux induisant des conséquences cardio-métaboliques. En utilisant de façon complémentaire des approches in vitro, des approches précliniques in vivo chez des singes macaques et des approches cliniques dans des cohortes de sujets vivant avec le VIH, ce groupe a pour ambition de comprendre la physiopathologie de l’atteinte du tissu adipeux et des complications qui en résultent. Il étudie le rôle respectif du virus qui produit des protéines virales délétères, des molécules antirétrovirales spécifiques et de l’association des deux.

L’exposition à long-terme aux glucocorticoïdes, qu’ils soient endogènes ou exogènes, est fréquemment associée à des modifications de la distibution du tissu adipeux qui conduisent à une insulino-résistance sévère, et à des altérations de l’homéostasie glucido-lipidique. Dans ce cadre, notre groupe réalise de façon complémentaire des études cliniques, des études précliniques avec la création et la manipulation de modèles transgéniques qui permettent de moduler la sensibilité aux glucocorticoïdes et des études in vitro avec des modéles cellulaires afin de comprendre la physiopathologie de cette LD ainsi que les adaptations métaboliques et endocrines qui en résultent. Notre travail se concentre en particulier sur le tissu adipeux et sur le pancréas endocrine.

Cet axe rassemble les expertises complémentaires de tous les investigateurs seniors de ce groupe en interaction étroite avec les membres des autres groupes de l’équipe. De nombreux éléments montrent que les LD génétiques et acquises ont des conséquences sur la fertilité, avec la survenue de syndromes des ovaires polykystiques et une anovulation chez la femme et des défauts de la qualité du sperme chez l’homme. Grâce à des modèles in vitro et précliniques in vivo, ainsi qu’à l’analyse de larges cohortes cliniques, ce groupe a comme objectif de comprendre les mécanismes responsables des dérégulations des fonctions reproductives résultant des atteintes du tissu adipeux observées chez les sujets lipodystrophiques.
